For the love of fleecy little lambs
Dear Medical Bureaucrats,
They are my immunization records. I do not have to explain to you why I need a copy of them, why I don't currently have the copy of them you allegedly gave me six years ago, or ritually decry my carelessness in needing a copy of the damn things. They are patient information. I am the patient. Give me the copies, for which I am paying, without the five minute lecture on what a fluffheaded child I am because I had to ask for them.
